Step 1
Design the first page of the book if your entry will be the first. Enter your last name, first name, patronymic and date of birth using your passport details. To fill in the items "Education" and "Specialty", take the corresponding document, indicate the type of education (higher professional, secondary specialized, secondary general, etc.) and specialty without specifying qualifications (teacher, economist, lawyer, accountant, etc.).). Put your signature with the decryption and seal of the individual entrepreneur legibly. The employee must also put a personal signature, certifying the data you specified.
Step 2
In the section "Information about work", start recording by filling in the third column of information about the individual entrepreneur (who works, surname, name, patronymic in full). For example, "Private lawyer Ivanenko Sergey Vladimirovich." On the basis of the employment contract, issue an order or order for the employment of this employee, make an entry for the next number in the work book. Put the date of admission indicated in the contract, not the date of filling! Make a full entry in the third column, indicating the position, department, etc. For example: “Accepted for the position of a salesperson”. In the last column, write down the type of document, its date and number ("Order dated 05.03.2001, No. 3"). Note that if the hiring was carried out before 06.10.2006, then the entry must correspond to the date in the employment contract.
Step 3
When dismissing an employee, put the following record number, the date of dismissal, the reason (ground) for terminating the employment contract in accordance with the Labor Code of the Russian Federation, indicating the clause and part of the law, the name of the document, its date and number. It is necessary to indicate the position of the filling person who signs and decrypts, then certifies this record with the seal of the individual entrepreneur. The familiarization of the employee is made against signature. If he was hired before October 06, 2006 and there is no corresponding entry in the work book, then the termination record after this date will not be valid! The last working day is the day of dismissal.
