Instructions

Step 1

On the first page of this form, a personnel worker writes the full name of the enterprise in accordance with the constituent documents, the enterprise code in accordance with the All-Russian Classifier of Enterprises and Organizations.

Step 2

A personal card is entered for an employee with the same number as this employee was accepted for the position. Therefore, the date of compilation corresponds to the date of hiring. The card is assigned a personnel number. The regular employee enters the taxpayer identification number and the number of the insurance certificate of compulsory pension insurance in accordance with the documents submitted by the employee. The personnel officer indicates the gender of the employee, the type of work (main, part-time). In the card, you must enter the number and date of the conclusion of the employment contract.

Step 3

In the T-2 card, the employee of the personnel department enters the full surname, name and patronymic of the employee accepted for the position, his date and place of birth, citizenship in accordance with the identity document.

Step 4

In accordance with the education document, the card indicates the status of education received during study at an educational institution, the series and number of the document, the start date and graduation date of the educational institution. If the employee has submitted several documents on education, the details of each document are indicated.

Step 5

The employee of the personnel department calculates the length of service of the accepted employee according to the work book and writes the number of years, months, days of work experience on the second page of the card.

Step 6

The card must indicate the employee's marital status (married, not married, married, not married), last name, first name, patronymic, date of birth of the next of kin. The personnel officer enters the series, number, date of issue, by whom and when the identity document was issued, information about military service.

Step 7

On the third and fourth pages of the employee's personal card, information about the transfer, certification, advanced training of the employee, awards for the time of work, vacation of the employee is indicated. When dismissing an employee, the date of dismissal, the number and date of issue of the order of dismissal are indicated. The employee puts his signature and the date of signing.
