Indicate the number of the dismissal order and the date it was drawn up. Write an article of the Labor Code for which you were fired. You need to write the amount of compensation that you were not paid and the period of unused vacation days. Indicate the date when the administration refused to pay you and the reasons explaining this decision. And also list the documents confirming the refusal. Indicate the actions that you consider illegal and the reasons for this opinion. You also need to list the evidence confirming the illegality of the actions of the former employer. Step 3
After all of the above, you need to fill out the section of the request to the court for compensation. Write the amount you want to receive. List possible witnesses and their addresses. Also include the address from where the court can request documents about your case.

The following documents must be attached to the application: a copy of the application, documents on the intended vacation and refusal of compensation, the decision of the labor dispute committee (if available at the enterprise).
