To fill out an application for deregistering a vehicle on your own, you must, firstly, know the name of the registration unit. These data can be found by phone or on the website of the traffic police in the section "traffic police in the regions". Next, you should indicate your last name, first name and patronymic and below describe the reason for the removal of the vehicle from the register (sale, disposal, etc.). After that, it is necessary to describe the documents that are attached to the application (receipt of payment of the state duty, vehicle inspection certificate, vehicle registration plates, vehicle registration certificate, identity document, registration document or technical passport of the vehicle)
Further in the form, at the very beginning of the frame, it is necessary to fill in the column "Information about the owner", in this column the personal data of the owner of the vehicle are filled in, such as: surname, name, patronymic, date of birth, identity documents, place of residence, citizenship, gender and TIN (if available).
After providing information about the owner, you must provide information about the vehicle. This data can be obtained from the documents for the vehicle. In this section, fill out: state registration number, body number, color, identification number (VIN), engine power, make, model, environmental class, permitted maximum mass, unladen weight, vehicle type, vehicle passport, manufacturer, category (A, B, C, D, trailer - E), year of manufacture, chassis (frame) number, registration document.
This is followed by the column "Representative of the owner", this column is filled in if the deregistration of the vehicle is made not by the owner, but by his representative. In this column, you will need to fill in: last name, first name, patronymic (representative of the owner), date of birth, identity document, address of residence, telephone and power of attorney (date, registration number, if any).
After filling in all the columns, it is necessary to put a number and a list, the date is better set by the number when the vehicle will be deregistered (if the application is filled out in advance).
