A passport is the main document of a citizen, proving his identity. According to the laws of the Russian Federation, every person from the age of fourteen is required to have a passport, otherwise he will have to pay a fine. But what to do in that situation if the passport is lost. It needs to be restored, and part of this process is writing a statement about the loss of the document.

Step 1
Prepare the required documents. Take passport-sized photographs. Also prepare documents for which the necessary marks are put in the passport - military ID, marriage certificate, birth certificates, and if you wish, you can provide a certificate of assignment to you of an Individual Tax Number (TIN) and a medical certificate of the group of shelter and Rh factor.
Step 2
Pay the state fee. For persons who have lost their passports, in 2011 it is 500 rubles. You can download and print a receipt on the website of the Federal Migration Service (FMS) and subsequently pay for it through any bank, or come to Sberbank and fill out a receipt according to the sample posted on a special stand in the branches of this bank. When filling out the receipt, indicate not only the details of the addressee, but also the purpose of the payment, as well as your personal data - last name, first name, patronymic and address.
Step 3
Come to the FMS branch at your place of residence with all the documents. Its address can be found on the official website of the organization, in the section "FMS of Russia", in the subsection "Local authorities of the FMS of Russia". Fill out an application for the loss of your passport on the spot, in which you must indicate how it was lost, as well as in what place and when. You also need to write a second application, already for the issuance of a new identity document. The documents must be filled in according to the sample that the migration service officer will give you.
Step 4
Do not forget to get a certificate confirming your identity at the time of registration of your passport. Also check when you can pick up the finished document. It usually takes one to two weeks to complete.
Step 5
After the required time has passed, you will receive your new passport. If this has not been done before, put in it a registration stamp with your management company.
