Step 1
Application for a new passport. The form of such an application and a sample of its filling are in the passport office.
Step 2
4 photographs of size 3, 5 x 4, 5 cm, black and white or color. Photos must be made on matte paper, on the back of each photo there must be the applicant's name, his surname and patronymic in full.
Step 3
A receipt for payment of the state duty for a passport to replace a lost or worn-out one. Such a receipt can be downloaded from the Internet, or obtained from the passport office before submitting documents. You can pay the state duty at any branch of Sberbank. Its value is 500 rubles. In addition, for the loss of your passport, you also face a penalty in the form of a fine - 300 rubles.
Step 4
Certificate of identity and citizenship of the Russian Federation, or any additional document that can confirm citizenship and identity: passport, driver's license, birth certificate, extract from the house book at the place of residence.
Step 5
To confirm the registration, you will need an extract from the house book at the place of residence (taken from ZhKO, REU, DEZ).
Step 6
You may need a marriage or divorce certificate and birth certificates for children under the age of 14. For widowers, you must bring a death certificate for your spouse.
Step 7
For men, you must have a military ID or a certificate from the military registration and enlistment office with you.
