Step 1
Get a passport form to fill out. This item can be purchased at the club where your animal is registered, or at the veterinary clinic.
Step 2
Start filling out the document. Indicate in it the name of the animal, its color, year of birth, breed (a mongrel animal is written as "mestizo").
Step 3
Give your passport to the clinic. It must make notes on vaccination and antiparasitic prophylaxis. These data can be transferred from the old animal card, entered in the clinic, or entered directly after medical intervention. The information must be indicated by the veterinarian and certified by the seal of the clinic. Only in this case it will be recognized as suitable for the issuance of documents permitting the export of an animal to another country.
Step 4
Fill out the section of your passport on the animal's reproductive situation yourself. This information is optional and will be useful, first of all, for breeders for storing data about a pet. In this section, the owner records the dates of mating of the animal and the birth of cubs, including their number.
Step 5
If you want to take your animal abroad, get a special certificate based on your international passport. It is valid for only a few days, unlike a passport. Please note that vaccination requirements may differ depending on the country where you are taking your animal. This information can be clarified, for example, at the consulate of the country you are going to, when obtaining a visa.
