- the reason for issuing this certificate (where it is provided);

- on the basis of what this certificate was issued (entries in the work book, personal file, archival data, data of a civil law contract, etc.).

A certificate of experience is certified by the signature of the director of the enterprise / organization, the chief accountant and the head of the personnel department with decrypted signatures and the required seals.
