According to the Federal Law No. 345-FZ of December 8, 2010, the right to receive a package of social services (social package) is granted to citizens assigned to federal beneficiaries. From January 1, 2011, all holders of social packages have the right to refuse them and receive a monthly cash payment.
Instructions
Step 1
Decide which part of the benefits package you are going to give up. According to the federal law, since 2010, the social package for beneficiaries consists of three parts: medicinal, sanatorium and transport. You can refuse any of them, for example, retain the right to free medicines, but receive monetary compensation instead of spa treatment. You can also replace the entire benefits package with monthly payments.
Step 2
Contact the branch of the Pension Fund of Russia at your place of residence and submit there a written waiver of the social package, drawn up in a standard form. Under the heading "Application for refusal to receive a set of social services (social services)", indicate your full name and passport information. If you are a legal representative of a minor or an incapacitated person, fill in the appropriate fields for yourself and the beneficiary you represent.
Step 3
Indicate: you refuse a full package of social services or ask to replace some specific social service (one or two) with cash payments. At the end of the document, put a date and a signature confirming that you agree with the change in the form of providing social services, and are also aware of the possibility of resuming the receipt of these services in the form of social benefits (this will require a new application, which is also written in the Pension Fund of Russia).
Step 4
Keep in mind that you will need to write an application and submit it to the Pension Fund employee personally, written refusals brought by neighbors, relatives, social workers, and so on, are not accepted for consideration. The exception is notarized statements or refusals that came from places of deprivation of liberty or hospitals. In these cases, the document must contain the certifying stamps and signatures of the head of the pre-trial detention center or a doctor.
