The need to obtain or replace a passport sometimes catches by surprise when the tour has already been paid for and travel tickets have been received. To prevent this from happening, take care of this in advance. And so that you do not have to stand in vain a rather large queue at the FMS department, find out in advance how to correctly fill out a questionnaire for a passport.

Step 1
To correctly fill out a passport application form, you need to enter the correct data into it. They can be found out from a citizen's passport or birth certificate and work book, if any. This can be done in two ways: by hand in legible writing in black or blue ink, or on a computer using a PDF creation form.
Step 2
Before you start filling out, remember that the questionnaire is an official document, so it cannot be corrected or corrected. If you spoil the form, take a new one. It is not necessary to go to the FMS department for it, you can print several copies at once by downloading the file from the Internet.
Step 3
It is possible to issue a passport with a chip for 10 years for an adult citizen over 18 years old and a child under 18. In the first case, the applicant himself writes the application for it, in the second - his legal representative, who may be a parent, adoptive parent or guardian. Thus, there are two forms of questionnaires. When the child reaches the age of 14, the passport data is indicated, up to this age - the birth certificate.
Step 4
Fill in the details: full name, gender, date and place (city) of birth. If there was a change of surname, then indicate when and where. In this case, in the second line of the first column, write “Until XXXX year,”. In the “gender” column, write the entire word: “female” or “male”.
Step 5
Indicate the address of residence or registration. In the "Citizenship" column, enter the name of the state, for example, "Russian Federation". Complete the second blank of this section for dual citizenship. If not, then insert "I have not".
Step 6
Fill in the details of your regular passport or birth certificate. When filling in columns 8 "Purpose of obtaining a passport" and 9 "Obtaining a passport", pay attention to the words at the bottom of each column in brackets. These are possible options for your answers to the relevant questions. Further, the forms of questionnaires for adult and minor applicants differ.
Step 7
For a citizen over 18 years old
If the answer is yes to questions 10 and 11, about possession of secrets and contractual obligations, indicate the name of the organization and the year of registration. In columns 11-13, answer “Yes” or “No” to the questions about the call to military service, conviction by the court or evasion of obligations imposed by him.
Step 8
In section 14 "Information about work activity", inform about the places of work, study and military service over the past 10 years. If there was a period in which you did not work, indicate the date of its beginning and end, and in the column "Address" indicate the address of residence.
If you already have a passport, then enter its data in column 15. Enter the date of filling out the questionnaire and sign in the "Signature" rectangle. By doing this, you confirm that you accept responsibility for the correctness of the information.
Step 9
For a citizen under 18
Write “Yes” or “No” in boxes 10 and 11 for conviction of a crime or evasion of court obligations. In column 12, enter the data of your passport, if any. Enter the date for filling. The child should sign in the rectangle at the bottom of the sheet.
Step 10
On the back, write basic information about yourself as a legal representative. This is the full name, including before the change, date and place of birth, residence address and passport data. Re-enter the date of completion and sign your own name.
