Every significant change in an employee's career, especially dismissal, must be legally reflected in his work book. If an employee leaves on his own, a note about this in his work book must reflect this circumstance and a link to the corresponding article of the Labor Code of the Russian Federation (part 3 of article 77) and be certified by the signature not only of the representative of the organization, but also of the employee himself.

- - fountain pen;
- - stamp of your organization or individual entrepreneur.
Instructions
Step 1
Ask the quitting employee to write a letter of resignation. This document is drawn up in the name of the head of the organization. By law, the employee must notify the employer of his dismissal fourteen days in advance, but he can specify a later date for the termination of employment. The application must be issued by the head of the organization, and in some cases before him - by the heads of intermediate links, starting with the immediate boss of the dismissed employee.
Step 2
Prepare an order to dismiss the employee based on his application. This document must have a number and date, be certified by the signature of the head of the organization or a person replacing him (for example, the deputy director, if the first person is on vacation, business trip or sick leave). In the order, indicate the date from which you dismiss the employee.
Step 3
Ask the employee to sign the order that he is familiar with this document. You can provide for this a place at the bottom of the order sheet - below the signature of the manager and the seal: the word "Acquainted", a place for the employee's signature and its decoding and date. But you can not do this: in practice, only the signature of the employee is enough.
Step 4
Make a note of the dismissal in the employee's work book. Give her a serial number (the most recent entry plus one) and indicate the date of dismissal indicated in the order.
Step 5
Fill in the field for job details. The optimal wording for this: "The employment contract was terminated on the initiative of the employee in accordance with part 3 of article 77 of the Labor Code of the Russian Federation."
Step 6
In the column on the far right, indicate the name of the administrative document on dismissal (order, it can be abbreviated), its number and date of signing.
Step 7
Certify the entry with the signature of an official (head of the enterprise, head of the personnel department or other employee with the appropriate authority) with the name of the position and the decryption of the signature and seal. All this is entered in the column for information about the work directly under the letter of dismissal.
Step 8
Ask the employee to sign under the dismissal record in his work book that he is familiar with the order. He must also do this in the section for information about work under the seal of the organization and the signature of its representative. The wording "familiar" and the decryption of the signature are desirable but not required. This completes the registration of the employee's dismissal.
