The main requirements for an entry in a work book about dismissal are the wording in strict accordance with one of the grounds provided for by the Labor Code of the Russian Federation, and a link to the corresponding resolution of the Labor Code of the Russian Federation. The entries must also be assigned a serial number and date and indicate the basis on which it was entered in the work book (order or other document with a number and date).

Resignation records of their own free will, like any other, are assigned a sequential number. It should be strictly one more than the previous mark.
The date of dismissal is usually the same as in the order about it. If this day falls on a day off, it is better to indicate the last worker before it.
In the column for information about hiring, transfers and dismissals in the same line as the number and date is written "Dismissed at will."

The name, date and number of the order or other administrative document on dismissal are entered in the last column.
The resignation record, unlike all the others, is certified by the signature of a representative of the organization with an indication of the position and a transcript (a personnel inspector or other employee who is entrusted with this function is enough) and a seal.
