Step 1
If a citizen works in one organization in two positions, he needs to write an application addressed to the first person of the company with a request to make an entry in his work book about part-time work. On the application, the employee puts his signature and the date of writing the application.
Step 2
The head of the enterprise issues an order on the basis of a statement about the possibility of entering his additional position in the employee's work book. The document is assigned a number and date. The director signs it and puts the seal of the organization.
Step 3
In an employment contract with an employee working in the main and additional positions, it is necessary to write that this work is a combination for him. Moreover, he can work on it only in his spare time.
Step 4
In the work book of a specialist, the personnel officer puts the number of the ordinal record and the date of hiring part-time after the entry about the main position. In the information about the work, the position, structural unit, in which the employee is admitted, is indicated. It is necessary to prescribe that this work is a combination for him. The basis for the entry is the order for admission to an additional position, the personnel officer writes its number and date.
Step 5
If an employee works in two positions in different organizations, he needs to take from the additional place of work a copy of the employment order signed by the director of the enterprise, an employment contract or a certificate on letterhead indicating that the employee really works in this company.

The personnel officer of the main place of work makes an entry in the employee's work book about hiring him for a part-time job, the basis of which is one of the submitted documents. Moreover, the full name of the organization, the name of the position and the structural unit of the additional place of work are indicated. The employee is recommended to keep the certificate on the letterhead of the part-time job in the work book and present it when required.
