Instructions

Step 1

In points 1-5 of the questionnaire, you must enter your basic data. In the first line, indicate the last name, first name and patronymic, in the second - data on their change, if you have not changed, then write "did not change (a)", if changed, then indicate the previous last name, first and middle name in full, the registry office, in which the change is registered and the date of registration of the change. If the surname has been changed several times, then indicate all cases of the surname change in the reverse order.

Write your date of birth in the format "01 March 1970". If the birthday number consists of one digit, then put "0", "g" or "year" in front of it, do not write. Indicate gender entirely "male" or "female", and rewrite the place of birth exactly from your passport of a citizen of the Russian Federation.

Next, indicate the place of residence, in accordance with registration, in the following order: zip code, country / republic, region, region, settlement, street, house, building, apartment, telephone.

Step 3

Clauses 6-9 relate to the issues of your citizenship and the purpose of obtaining a passport. Write citizenship in the accusative case - "Russian Federation". Next, answer the question about having a second citizenship, if you don't have one, write "I don't have", if you have - indicate which state you are still a citizen of. Enter the data from your passport of a citizen of the Russian Federation.

Step 4

On the next line, you must indicate the purpose of obtaining a passport. For example, if you need a passport for tourist trips, then write "for temporary trips abroad." If your previous passport has expired, then write “instead of the used one”, if you receive it for the first time - “primary”. To obtain a passport "in exchange for the lost" you will need a certificate from the police about the loss of your passport.

Step 5

In paragraphs 10-13, it is necessary to answer the questions related to the possibility of obstacles to travel abroad. If you did not have access to information of national importance, then write “was not”. In the same paragraph, answer the question about the presence of obstacles to travel abroad. If there are none, then write “I have not”.

Step 6

Indicate the presence of a draft for military service - "not called (a)" (women should also write).

If there are no convictions or charges, write “not convicted (a)”. If you have obligations imposed by the court, then you do not need to indicate which ones, it is enough to write "I do not evade", if there are no obligations, then also write "I do not evade."

Step 7

To fill out point 14, you will need information from your work book.

Please provide information about your work over the past 10 years. Dates are written in the format "03.2007" (ie month and year), if the day of the month consists of one digit, then put "0", "g" or "year" in front of it. In the column "Address of the organization", be sure to indicate the city. If you had a break from work or study for more than one month, be sure to write "did not work (s)". In this case, in the column "Organization address" indicate the address of registration at this time. Upon completion of filling out this item, put the date of filling (the month is written in full), the surname and initials of the person authorized to certify such information at your place of work. The data from the work book must be certified by the signature of the specified person. If you do not work, then the data is not certified by anyone, and the original work book serves as a confirmation.

If you did not have enough lines for a list of all places of work, then write down the continuation of your work activity in the "Appendix to the application for the issuance of a passport."

Step 8

As the last point, enter the data from the previous passport. If you are getting a passport for the first time, then write “I have not”.
