Death is a sad phenomenon, but, unfortunately, inevitable. If you have to bury a loved one, relative or just an acquaintance, it will be necessary to draw up a number of documents. Be sure to take care of obtaining a death certificate. There are two types of certificates in total: a medical death certificate and a stamped death certificate.

Step 1
A medical death certificate in the form No. 106 / u-08 is a medical statement of the death of a person. It is issued for statistical purposes and to ensure the state registration of death. If you do not have this certificate, the body of the deceased from the morgue will not be handed over to you.
Step 2
Family members of the deceased can obtain a medical death certificate. If there are none, then a certificate is issued to the guardian (legal representative) or close relatives of the deceased (deceased).
Step 3
A certificate is issued where the person died: in a dispensary, maternity hospital, clinic, etc. To obtain a medical death certificate, you will need to present: - the passport of the deceased; - his outpatient card; - health insurance policy of the deceased; - your passport (applicant).
Step 4
When receiving a medical certificate, be sure to check: - whether the date of death and the date of issue of the document are written correctly; - whether the entries made in the certificate correspond to the passport data; - whether there is a record of the place of death; - whether there is a round stamp of the medical institution on the back of the document, signature, surname and position of the doctor who issued the certificate, diagnosis.
Step 5
If the medical certificate of death is marked "preliminary", it means that additional research will be required to clarify or establish the cause of death. After additional studies have been carried out within 45 days, a certificate is issued with the note "in lieu of the preliminary". It bears the number and date when the previous certificate was issued.
Step 6
If a medical death certificate is lost, it is quite difficult to recover it. If restoration turns out to be impossible, then the state registration of death is carried out on the basis of a court decision on establishing the fact of death.
Step 7
You can get a stamped death certificate from the registry office. To do this, you will need to provide a number of documents: - passport of the deceased; - medical death certificate; - applicant's passport; - if a person acts by power of attorney from close relatives of the deceased, then he will need to provide a notarized power of attorney.
Step 8
A death certificate is issued free of charge on the day of application.
Step 9
For state registration of a stillborn child, you must present a document of perinatal death of the established form. In this case, a death certificate is not issued. At the request of the parents, a document is issued that confirms the fact of state registration of the birth of a dead baby.
Step 10
In the event of the death of a child during the first week of life, state registration of his birth and death is carried out on the basis of birth documents and perinatal established death. In this case, only a death certificate is issued.
Step 11
If you lose the stamped death certificate, you must contact the registry office that issued the previous certificate, with an application for a duplicate. You will need to have your passport with you and a copy of the lost certificate, if any.
