Any change in the terms of the contract must be recorded by the parties in a supplementary agreement. It is drawn up in writing, one copy for each of the parties to the agreement.
Instructions
Step 1
Give the supplementary agreement a number and date it was written. Indicate to which document the agreement is being written, refer to the number of the original agreement and the date of entry into force. You can also give a name to the document itself, for example, "Supplementary agreement on changing the sum insured" or "Supplementary agreement on changing the timing of delivery of materials."
Step 2
Indicate in the header of the letter the names of the companies-parties to the agreement, their organizational and legal form, as well as the names, surnames, patronymics of the people who will sign the additional agreement. Refer to the document on the basis of which these persons act. For example, "represented by the General Director acting on the basis of the Charter" or "represented by the Executive Director acting on the basis of power of attorney 55 dated 05.05.2010".
Step 3
Write down what changes in the terms of the contract the parties agreed on. If the agreements reached relate to several places in the original document, formulate the new editions in separate numbered paragraphs. For example, “the parties have agreed to read clause 6.1. of the agreement in the following version … "," the parties agreed to exclude clause 4.3 of the agreement from consideration "," the amount of the additional insurance premium is set at 10,000 (ten thousand) rubles."
Step 4
Write down that the supplementary agreement is drawn up in two (sometimes in three) copies - one for each of the parties. Note the date this additional agreement becomes effective.
Step 5
In the last paragraph, indicate the details of the organizations that are parties to the contract and agreement. Sign with the authorized person on whose behalf the supplemental agreement was drawn up in your organization and stamp it. Submit documents for signature to the counterparty.
