The work book is the main document, which is a confirmation of the work activity and length of service of the employee. In order to avoid problems with paperwork in the future (for example, upon retirement), all data in the work book must be entered correctly and competently.
Instructions
Step 1
According to clause 26 of the "Decree of the Government of the Russian Federation" dated 04.16.2003 No. 225, the change of the surname in the employee's work book is carried out by the employer on the basis of the documents provided (passport, marriage certificate, divorce certificate).
Step 2
The change of the name in the employee's work book is made on the basis of a statement written by him personally to the name of the head of the organization, which must contain a request for changes, indicating the reasons and attaching the relevant documents.
Step 3
Based on this application, the organization issues an order in any form to change the name of the employee. This order, together with copies of documents (passport, marriage certificate, divorce certificate), are included in the employee's personal file.
Step 4
Changing the name of an employee is primarily carried out on the title page of the work book. The old data is neatly crossed out with one straight line so that it can be easily read. New data is indicated at the top.
Step 5
On the inside of the cover of the work book, a detailed description of the changes made is made. That is, a record is made about the documents on the basis of which the changes were made. Below is the position, signature and decoding of the signature of the responsible person, which is certified by the seal of the organization or the seal of the personnel department, if changes are made by a personnel specialist.
Step 6
Under the entered data, the employee who owns the work book puts the entry "Acquainted (s)", the signature and the decryption of the signature.
