In order to reward their employees, the employer should draw up an order for bonuses in the unified form T-11 or T-11a (depending on the number of employees who need to be encouraged). Step 1
The head of the structural unit draws up a memo addressed to the first person of the company, in the document indicates the last name, first name, patronymic of the employee who should be encouraged, the position he occupies in accordance with the staffing table. Fills in the reason why it is necessary to reward this employee, and also writes the amount of the bonus money or percentage of the salary. The head of the structural unit puts a personal signature and decryption. The memo is sent to the director of the enterprise for consideration. In case of a positive decision, the head of the company puts a resolution on it with the date and signature.
Step 2
The basis for issuing a bonus order can be a characteristic of an employee of the organization, in this document it is necessary to briefly describe the employee's labor activity, the achievements of a specialist in this industry. The document is signed by the head of the structural unit and sent to the director of the enterprise.
Step 3
In the unified form T-11 or T-11a of the order, enter the full and abbreviated name of the enterprise in accordance with the constituent documents or the surname, name, patronymic of an individual in accordance with an identity document, if the legal form of the company is an individual entrepreneur. Write the title of the document in capital letters, give it a number and date of publication. Indicate the subject of the order, which in this case corresponds to the encouragement of the employee of the organization, as well as the reason why it is necessary to reward this employee. The reasons may be the anniversary of professional activity, the anniversary of the organization, and others.
Step 4
In the administrative part, enter the last name, first name, patronymic of the specialist who should be encouraged, his personnel number, the position held in accordance with the staffing table, as well as the amount of bonus money or percentage of the salary, which will be the amount of the bonus if the order is issued for one employee … If the document is drawn up in the T-11a form for two or more employees, then similar data for specialists and the amount of incentives should be entered in the tabular section.
Step 5
The director of the enterprise has the right to sign the order. Certify the document with the seal of the organization, familiarize the employee (workers) with it against signature.
