Step 2

A request for a certificate can be submitted both orally and in writing. In this case, you must have an identity document with you (for example, a passport of a citizen of the Russian Federation).

Step 3

The procedure for filling out the form is as follows. On a blank form, indicate the exact date of issue of the certificate. It is desirable to indicate the month in words. Next, write: "Issued gr." And indicate the last name, first name and patronymic in the genitive case without abbreviations. Enter the reference number below. It must match the one entered in the accounting log.

Step 4

Enter the registration address of the family you are providing. Now go to the "Family Composition" column. Indicate each family member, his last name, first name, patronymic, date of birth. Be sure to write down the degree of relationship with the person to whom the certificate is issued. Family members must be numbered starting with "1". If after filling there are empty spaces, put a flourish Z.

At the bottom of the certificate, write: "The certificate is given for presentation at the place of demand." Now below, indicate the surname and initials of the director (head) of the organization that issued the certificate. Nearby must be his signature, certified by the seal.
