Instructions
Step 1
Indicate the date of issue of the certificate (arrow 1): day, month (in words) and year. In the field "Issued gr." (arrow 2) write in the genitive case without abbreviations the surname, name and patronymic of the person for whom the certificate is being issued. In the field (arrow 3) located under the date of issue, indicate the number of the certificate under which it is registered in the journal of outgoing correspondence.
Step 2
In the fields marked with arrows 4 and 5, enter the address where this family is registered. For example: st. or Svobody Ave, house 34 sq. 8, if the place of residence is a private house in the "apt." put a dash. In the line "Family composition" indicate the number of registered people, this record should look like this: registered N (1, 2, 3 …) people. Below, line by line indicate all family members (arrow 6) and their years of birth, and also indicate who they are to the owner of the certificate, for example: Ivanova N. T. 1980 - daughter. If there are blank lines on your letterhead, be sure to cross them all out with a capital Z.
Step 3
Having indicated all the necessary information about the family, proceed to filling out the bottom of the certificate (arrows 7, 8 and 9). In the field "The certificate is given for presentation" write: at the place of request. The next line reflects data on the head or director of the institution to whom this certificate is issued. Indicate the surname and initials of the chief (director), also, if necessary, the number of the site. The last line (arrow 9) - "Passportist", it is necessary to enter the surname and initials of the passport officer in it, next to which (or in front of them) there must be his (her) signature. After filling out, go to the head, and confirm the authenticity of the entered data with an imprint of the company seal of the organization, providing all the necessary documents for this (house book, passports of residents and birth certificates).
