Enter the last name, first name, patronymic of the employee in accordance with the identity document. Indicate the period of work of this citizen in your organization, write the date of admission and the date of dismissal in accordance with the entry in his work book.

Indicate the number of calendar weeks in the last twelve months preceding the actual date of termination of this employee.

Step 5

Write the names of the last three months. In accordance with the payroll for this employee, enter the amount of earnings for the indicated months. For each month, indicate the number of working days according to the plan for this employee, as well as the number of days actually worked. Indicate the number of days of absence of the employee on the certificate of incapacity for work, if any. Enter the number of vacation days if the citizen went on vacation during the required period. For each column, calculate the totals by adding the numbers for the last three months.

Calculate the employee's average daily earnings for the last three months. To do this, add up each month's wages and divide by the total number of days worked in the previous three months. Enter your result in the appropriate field.

Step 7

Calculate the average monthly earnings, divide the total amount of the employee's salary by the total number of days actually worked. Multiply this result by the total number of days actually worked divided by three. Enter the average monthly salary in numbers and words.

Step 8

If wages were not paid to the employee during the specified period, indicate this fact, enter the name of the month and the reason for non-payment.

Step 9

Indicate the number of the current account or payroll, on the basis of which this certificate was issued.

Step 10

The document is signed by the head of the organization and the chief accountant, indicating their names and initials. Verify the certificate with the seal of the company, indicate the contact phone number of the company.
