Determine the employee's average monthly earnings. To do this, add up the cash payments to the employee for the last 12 calendar months. Their amount includes bonuses, salary, additional payments. The days when the specialist took unpaid leave, skipped or could not work due to the announcement of downtime due to his own fault should be excluded. In accordance with the production calendar, divide the received amount by the number of working days for the given period. Thus, you will calculate the employee's average daily earnings. Multiply it by the number of days actually worked in that particular month. Write the result in the appropriate box.

Certify the certificate for the employment center with the signature of the chief accountant, director of the enterprise (indicating their positions, surnames, initials), with the seal of the company. It should be borne in mind that the information specified in this certificate is the responsibility of the above-mentioned persons. If they do not comply, they can be punished in accordance with the legislation of the Russian Federation.
