Is your man unwilling to acknowledge his child? In this case, you can apply to the court for the establishment of paternity for the recovery of alimony. Please note that the text in the application form is written only in block letters.
Instructions
Step 1
Write in the upper right corner where exactly the document is going and who is its author. In this case, indicate in the top line: "B (name of the court)", and on the next line: "Plaintiff: (your name, surname, patronymic, date of birth)". Please write your address below, and below: "Respondent: (details of your child's father)", and the address where he lives.
Step 2
Step back the line and write the name of the document in the center. In your case, it is called the Paternity Claim.
Step 3
Below, from the red line, write a statement with the following content: “With the defendant (full name and date of birth of your child's father), I was in a de facto marriage relationship (if you were married) from (indicate the date, month, year of marriage) to (date, month, year of divorce).
Step 4
Step back the line and write: "(full date of birth of your child) I gave birth to (daughter or son) (surname, name and patronymic of the child)." From the red line: "The defendant is (his or her) father, but the application for registration of paternity with the registry office, which is confirmed by evidence, refuses to submit."
Step 5
Next, in the column, list all sufficient and necessary evidence for the court, confirming the fact that the defendant is the father of your child. If you were married or are in a marriage with the defendant, confirm that you are living together and running a common household. Attach a marriage certificate, personal photographs, letters confirming the fact of paternity, testimony of witnesses to the application.
