To obtain a passport, you must fill out an OVIR application form. From April 01, 2010 it can be filled in and sent by e-mail. The issuance of passports is regulated by the federal law "On the procedure for leaving the Russian Federation and entering the Russian Federation" "337-FZ".
Instructions
Step 1
You can fill out the questionnaire by hand, in this case, use only a black ink pen, write legibly, without errors or corrections.
Step 2
Fill in the full name, if the last name has changed, indicate this. Next, indicate the date of birth, gender, place of birth, place of registration (permanent residence), citizenship, passport data.
Step 3
In paragraph 8, indicate the purpose of obtaining a passport, in paragraph 9 - the initial receipt, in exchange for the lost, etc. When filling out the information in paragraph 10-13, indicate whether you have been admitted to information that belongs to state secrets; write information about military service; the presence of a criminal record or a recognizance not to leave. If the applicant is under investigation, convicted, then he will not be able to obtain a passport.
Step 4
Include information about the child. If you are receiving a biometric passport, this section does not need to be completed. All children from 1 month old are given their own passports. In the next paragraph, write information about your places of work for the last 10 years, at the end of the paragraph the number and series of the work book are indicated.
Step 5
Sign and date at the end of the questionnaire. Certify the document at the place of work, if you are studying, then at the place of study. A photograph is glued to the upper right corner, which is stamped on its lower left corner at the last place of your work. Remember that when filling out the questionnaire, all responsibility for the accuracy of the specified information lies with you.
Step 6
