Filling out a special questionnaire is required to obtain a foreign passport, and it does not matter whether you are issuing it for the first time or already again (instead of a damaged or expired one).
Instructions
Step 1
Before sending the questionnaire, you must fill in all the required fields (not a single blank column should remain). In addition, according to the latest requirements, it is not allowed to complete the document by hand or make any corrections to it. All data must be printed. Do not forget that the application is drawn up strictly on one sheet, and not on two different ones (that is, the questionnaire is printed on both sides of the sheet).
Step 2
In the column "Surname, Name, Patronymic" all data is indicated in full, and not in an abbreviated form. By the way, there is another blank line under the last name. There you must indicate whether you have previously changed your last name or not. If not, write the following: “Full name I have not changed (a). " If any changes were still made, indicate your previous surname, until what time did you wear it and the place where you changed it. For example: Petrova until 2005, Moscow.
Step 3
Information about the place of birth is entered only on the basis of the data of the civil passport (without adding additional data). Filling in the column "Place of registration or permanent residence", indicate the city, postal code, district, street, house and building, apartment, telephone, as well as the date of registration in the format "day, month, year".
Step 4
Do not forget to indicate your citizenship (if you have dual citizenship, enter that too). If you have no other citizens besides Russian citizenship, just write: I have not.
Step 5
The next column indicates the passport data. Enter them carefully, take your time, otherwise you will have to fill out the questionnaire anew. Next, clarify for what purpose you are getting a passport: for permanent residence abroad or for temporary trips.
Step 6
Please note: each completed application must be certified with a seal (it is placed on the back). Working citizens certify the document at their place of work. This is done by the head or his deputy. Those who are unemployed or retired should not go through this procedure. Students will need to put a seal in the dean's office of their educational institution.
Step 7
After filling in all the fields of the questionnaire, take the document to the local branch of the Federal Migration Service. As a rule, each department has a website where you can find out about the location of the service and its schedule.
