Step 1
To issue a health book, first of all, you need to find out in which clinic there is such a service. Find out the list of necessary documents and analyzes that need to be collected. Bring your passport (original and photocopy), one 3x4 photo, vaccination data (if any).
Step 2
Come to the clinic for tests and medical examination. Be sure to do a fluorography and go through all the doctors recorded in your health record. They must diagnose you that you are healthy and fit for work. The prophylactic examination should include studies for tuberculosis, skin diseases, carriers and pathogens of intestinal infections, for helminthiases and other viruses.
Step 3
Write an application for the issuance of a form of a sanitary book, pay for it and a medical examination at the cashier's office of the clinic. Then you can start the inspection. Secure the prophylactic examination with the seal and signature of the head physician. All data of the medical examination will be stored in the database of the polyclinic in case of loss of the health record. Remember that recertification must be completed annually. That is, every year you have to come to the clinic, which issued you a health certificate, to undergo a second professional examination. Thus, you extend your health book, and at the same time confirm your fitness for work.
