Step 1
Determine the reason why the person may be deprived of parental rights. The legislation provides for six such reasons. This is a regular evasion of the duties of a parent (including the payment of alimony), refusal to take your child from a medical, educational or any other institution without a good reason, abuse of parental rights, alcoholism or drug addiction, child abuse, attempted death or death child or spouse.

Write the name of the court to which the application is submitted, then the name, surname, patronymic and place of residence of the plaintiff and the defendant. In the event that the defendant's place of residence is unknown, either his last known address or the place where his property is located should be indicated. You also need to indicate which third parties are involved in the case. This can be a prosecutor, employees of the guardianship and guardianship authority, etc.
Step 3
Indicate what exactly is the violation of parental rights by the defendant, what evidence you have of his guilt. Describe everything in detail, because your goal is to convince the court that you are right. For example, if the respondent is a chronic alcoholic or drug addict, it is necessary to provide as evidence the relevant certificates from medical institutions. If, however, the respondent evades his parental responsibilities, please describe in detail how this is expressed.
Step 4
Formulate a request to deprive the defendant of his parental rights, and to transfer the child to foster care to another person you have indicated. You can also demand to collect alimony from the defendant and indicate the amount that he is obliged to pay. Attach documents and evidence to prove your case.
