- name of the document ("power of attorney"), date and place of its preparation;

- information about the principal and the representative, indicating their coordinates and passport data;

- the content of the representative's powers in terms of receiving pensions (place, amount, the right to sign on settlement documents, etc.);

- the validity period of the power of attorney;

- personal signature of the principal;

- sample signature of the representative.

The power of attorney is drawn up in a simple written form, certified by the head physician of the hospital where the pensioner is located, and sealed with the seal of the medical institution. When receiving a pension, the representative will also need to have a document proving his identity.

It should be noted that the law does not limit the minimum and maximum duration of the power of attorney. However, if this requisite is not specified in the power of attorney, then it is valid for 1 year from the date of its issuance. After being discharged from the hospital, the principal has the right to early revoke the issued power of attorney. The representative can also refuse to receive the pension at any time. In this case, you need to issue a new power of attorney, but for a different person.

If a person receives a pension on a bank card, then you can transfer it to close relatives or other trusted persons, while providing a pin code. In this way, it is possible to withdraw money through an ATM. If you need to receive a pension directly at a bank institution, then you still cannot do without a power of attorney.
