Step 1

For registration of the characteristics on VTEK or ITU, use a special form. Step 2

In the questionnaire part of the characteristic, give a brief description of the employee's previous employment. If he worked in enterprises associated with difficult working conditions or increased danger, reflect the terms and periods of such work, indicate the names of the enterprises where he worked.

Step 3

Since this is a characteristic for physicians who will determine how much a person's health will allow him to continue his labor activity at your enterprise, give it a hygienic characteristic of his working conditions. Write down the position he occupies and give an assessment of his main job responsibilities, and also reflect the mode of his work.

Step 4

Write in the description how the work of this employee is related to labor productivity, and give a quantitative estimate of the production rates. Reflect in it if his work is connected with travel on business trips and how often he has to do it.

Step 5

Write if it is possible to retrain this employee and whether it is possible to transfer him to work with lighter working conditions. If there is a possibility of establishing a part-time or working week, also reflect it in the characteristic.

Step 6

Put down the approving signatures of the doctor of your company, the head of the legal department and the head of the personnel department on the document. Certify the document with the signature of the head and put the seal of the organization.
