The abbreviation UTII means “unified imputed income tax for certain types of activities”. When switching to this type of taxation, you need to write an application to the tax inspection body. The rules for submitting and writing such an application are governed by Art. 346.28 of the Tax Code of the Russian Federation. There is an order of the Federal Tax Service of the Russian Federation of 12.01.2011 No. ММВ-7-6 / 1 @, which sets out the procedure for writing an application for registration of individual entrepreneurs as payers of UTII.

Instructions
Step 1
The application form can be given to you by the tax office upon personal contact. You can also download it from the official website of the Federal Tax Service of the Russian Federation or find it freely available on the Internet. You must attach a copy of the certificate of registration of a legal entity or individual entrepreneur to the application.
Step 2
Places in the application that require filling are presented in the form in the form of squares. There are strict rules for writing an application by hand: you need to write with a black gel pen, always in block letters. The written text must be legible. Otherwise, the application will not be accepted and you will have to rewrite it.
Step 3
The information that must be submitted in the application is indicated in the Certificate of Registration of an Individual Entrepreneur (or legal entity) and an extract from the Unified State Register of Legal Entities. Name data must be written in separate lines intended for this. The page numbering in the application can be put down at the end of the letter.
Step 4
The second and subsequent pages are filled in by analogy with the title page. Be sure to indicate your contact phone number, so that in case of misunderstandings you can be quickly contacted. Remember that corrections and the use of corrective agents are not allowed.
