Step 1

A duplicate of the work book must be drawn up within 15 days from the date you filed an application for its loss. In the application, you should not only state the request for a duplicate, but also give detailed explanations under what circumstances the document was lost.

Step 2

If before employment in this organization you have already worked somewhere, then when filling out a duplicate work book, the employer must enter in the section "Information about work" (column 3) a record of your work experience (general and / or continuous) before entering this institution. This information must be confirmed by the relevant documents, so you must provide them (certificates, copies of orders, etc.) The total work experience is recorded in total, that is, the total number of years, months, days of work is indicated, while organizations, periods and employee position.

Step 3

Next, the total or continuous work experience is recorded, which must be confirmed by duly executed and provided by you documents, for individual periods of work, observing the following procedure:

- Column No. 2 indicates the date of employment;

- column No. 3 informs about the name of the organization where you worked, indicates the structural unit and the position held for which you were accepted.

If the documents you provided confirm the fact that you were transferred to another job in the same organization, a corresponding record is made about this.

Step 4

Information on dismissal (termination of the employment contract) is filled in duplicate in the following order:

- column No. 2 indicates the date of termination of the employment contract (date of dismissal);

- in column No. 3 - the reason for the termination of the employment contract, if the documents provided contain such information;

- column No. 4 indicates the name, date and number of the document confirming the corresponding entries made in the duplicate.

Step 5

In the case when the available documents do not fully provide information about the past work, only documented information is entered into the duplicate of the work book.

Step 6

The original documents confirming your work experience, after being copied and certified by the employer, must be returned to you. The list of these documents is not defined by the legislation. These may include copies of orders, certificates from previous jobs.
