The work book is the main document containing information about the employee's labor activity (Article 66 of the Labor Code of the Russian Federation). Among other information, it contains data on the enterprises at which this or that person worked, the period of work, the dates of admission and dismissal, the reasons for dismissal.
Instructions
Step 1
Based on the entries in the work book, at the end of the employment, the employee is assigned pension payments. Therefore, the loss of a work book leads to a loss of work experience and incorrect calculation of the pension.
Resolution of the Council of Ministers of the USSR and the All-Union Central Council of Trade Unions of the USSR No. 656 of September 6, 1973 "On work books of workers and employees" and "Instruction on the procedure for maintaining work books at enterprises, institutions and organizations", approved by the Resolution of the State Committee of Labor of the USSR of June 20, 1974 No. 162 (as amended on October 19, 1990), determine the procedure for the production, storage, maintenance and accounting of work books at the enterprise, as well as the issuance of their duplicates.
Step 2
A duplicate of a work book can be issued instead of a lost, worn out book, as well as at the request of an employee if an error or inaccuracy was made in the records.
Step 3
An employee who has lost a work book or an insert to it must inform the administration of the enterprise about this. In no more than 15 days a new book should be issued to him.
In all cases, in the upper right corner of the first page of the new work book, make the inscription "Duplicate".
Step 4
Then fill in the individual information about the owner of the work book based on passport data.
Entries in the duplicate of the work book are made in accordance with the general rules for filling out and on the basis of documents provided by the employee. These can be certificates and extracts from orders, statements for the issuance of wages, written employment contracts and other documents confirming work experience.
Step 5
The length of service, confirmed by documents, should be entered for individual periods of work: - in column 2, the date of employment is indicated;
- in column 3, the name of the enterprise is written, as well as the department (shop) and the position for which the employee was hired, records of transfers at the same enterprise;
-then below, in column 2, the date of the employee's dismissal is recorded;
- in column 3 - the reason for dismissal;
- in column 4, the name, date and number of the document are entered, on the basis of which the entries in the duplicate are made.
Step 6
In the duplicate of the work book, enter only the information available in the documents. Then return the documents that were used to confirm the length of service to the owner.
