Instructions

Step 1

The employee must write a statement addressed to the first person of the company with a request to give him a duplicate instead of the lost work book, put his signature and the date of its writing on the application. The director examines it and, in case of consent, affixes a resolution on it, signs and certifies with the seal of the organization.

Step 2

The head of the enterprise issues an order to issue a duplicate to the employee and fill it out by personnel officers. The document is assigned a number and date. It is certified by the seal of the company and the signature of the director. In the administrative part, the first person of the company indicates the last name, first name, patronymic of the employee, the position he holds, the name of the structural unit where he is registered.

Step 3

On the title page of a blank work book, write the word "duplicate", indicate the surname, name, patronymic of the employee, date and place of birth. Enter the status of the education you received during your studies (secondary, specialized secondary, secondary vocational, higher). Calculate the total length of service of the employee before joining you and indicate it.

Step 4

Based on supporting documents (orders, certificates) issued from the specialist's previous workplaces, write in chronological order the dates of the employee's admission and dismissal, in the information about the work the name of the organizations in which the employee was registered, in the fourth column - the numbers and dates of the relevant documents. Certify every record with your company's seal. Write your position, surname, initials, put your signature. Familiarize the employee with each signature.

Step 5

If one of the supporting documents contains incomplete information, an entry in the duplicate of the work book is not made.

Step 6

Issue a duplicate of the work record book to the specialist who has lost the original within fifteen days from the moment he wrote the corresponding application.
