Start filing a statement of claim. In the heading, indicate the name of the judicial authority to which the document will be submitted. This will speed up the proceedings in the court. Next, indicate your requirements that the respondent must fulfill, and the timing of their implementation. If the document concerns one or another financial dispute, indicate the value of the claim, the amount of collected or disputed funds. Personal signature and seal of the organization if it is filing a claim. Attach materials to your claim that can prove you were right, for example, contracts concluded with the insurance company, receipts, recorded telephone conversations, etc.

Gather the documents that should accompany your statement of claim. Submit an application to the judicial authority at the location of the insurer. You can file a statement of claim in person with the judge or his secretary at set hours, and also send it to the mailing address of the court.
