Step 1

The front side of the sick leave is filled in by the doctor of the medical institution. They include the personal data of the person being treated, his age, the name of the company where he currently works. The name of the medical organization is written (or its stamp, if available), its code is indicated, and the seal is affixed.

The sick leave indicates the periods of the citizen's incapacity for work. Each of them is certified by the signature of the attending physician (indicating his specialty and surname). The date from which the employee must start performing his job duties is written in words. When a person is sent for treatment to another medical institution, the latter gets a new certificate of incapacity for work. On the completed sick leave, the continuation number is entered.

The reverse side of the sick leave is completed by the employer. The timekeeper or personnel officer enters the name of the company, department, service where the employee is registered. The period of disability of the specialist, the date of going to work after sick leave is indicated.

Step 4

The personnel officers calculate the duration of the employee's insurance experience. It includes the periods of work of a specialist, during which employers made deductions of insurance premiums. The number of full years, months and days of the employee's experience is entered. The count is certified by the signature of the head of the personnel department.

Step 5

The payroll accountant writes in% of the employee's salary (tariff rate). It depends on the length of service of the employee. If the length of service is less than a year, then the allowance is calculated based on 30%, if from 5 to 8 years - from 50%, if from 8 to 10 years - 80%, more than 10 years - 100%.

Step 6

The average daily earnings of an employee is determined by dividing the actual earnings for the billing period by the number of working days in this period. The amount of the allowance depends on the daily wage (tariff rate) and the number of calendar days of the employee's incapacity for work. The allowance cannot be lower than the established minimum wage, which is regulated by law.

Step 7

The sick leave is paid partly at the expense of the employer, and partly from the social insurance fund. The total amount fits into the certificate of incapacity for work, the month is written in the payroll for which it is included. The sick leave is certified by the signature of the chief accountant (with the date indicated).
