Step 1

There is no unified form for an extract from the document, therefore, when drawing it up, be sure to follow the regulations and internal documents of the organization. In the main document, select those fragments of text that you want to see in the statement. If you want to retype it in its entirety, you don't need to select anything. On a blank sheet of paper at the top center of the document, type the name of the organization in full. Slightly below indicate the name of the document, for example, "Extract from Order No. 12 dated 01.02.2012".

Rewrite the exact wording of the copied document. For example, if this is an order, you must rewrite the statement of the fragment, not forgetting about the word "I order". If you are drawing up an extract from the protocol, write in the word “Decided” or “Decided”. If you are copying only a few paragraphs, be sure to keep their numbering (even if continuous numbers appear). Remember that you must completely reproduce the original text, without adding any words, phrases or even changing the ending, that is, you must quote the document.

The main text must be followed by the signature of the person who previously signed the main document. If you draw up an extract from the protocol, then it must be signed by all employees, whose signature is on the original document. Just below, certify all the information with your signature, indicate your surname, initials and position. Do not forget to affix the seal of the organization. If the document is required for internal use, it will be enough to put a stamp for registration of documents.

Be sure to certify the document with the secretary or other authorized person, he must write "Correct", indicate his initials and sign. Without this wording, the document will be invalid.
