This article gives an example of drawing up an application for an extract from the Unified State Register of Legal Entities (USRLE), which is usually used when applying to the tax authorities in Moscow.
As practice shows, an extract from the Unified State Register of Legal Entities for a certain organization is one of the most important documents, based on which, it can be concluded that the organization is in the register of operating legal entities, its governing bodies, registration with certain funds, etc. In this regard, the majority of legal entities monthly orders this document for themselves (to present it to counterparties), as well as for their main partners, in order to be aware of their possible liquidation, reorganization, or amendments to the constituent documents, or information contained in Unified State Register of Legal Entities.
The procedure for filling out this application is quite simple, and boils down to the following steps.
1. In the upper right corner, indicate the name of the tax office to which we submit the application.
2. Further, in the center of the page, we indicate the following phrase: "APPLICATION for the provision of an extract from the Unified State Register of Legal Entities (USRLE)".
3. Then, we indicate the full name, address of location, TIN, KPP, OGRN of the organization on whose behalf an application is submitted for an extract from the Unified State Register of Legal Entities.
4. After that, we indicate the organization in respect of which it is necessary to provide data from the Unified State Register of Legal Entities.
5. And, finally, at the bottom of the page, we sign this document by the head of the organization, and affix the seal and the number of drawing up the application.
