Free medical care is provided in our country on the basis of a medical insurance policy. Insurance for unemployed citizens is provided by local authorities, and the employer pays contributions for employed citizens. The medical policy has a certain period of validity, after which the document must be renewed.
Necessary
- - the passport;
- - employment history;
- - insurance certificate.
Instructions
Step 1
Apply to the HR department of the company or institution in which you work. Show your old insurance policy and ask for a replacement, extending the validity period. The employer must issue a new document within up to five days. In the absence of a separate HR department, the responsibility for issuing a policy is usually assigned to the HR manager or chief accountant.
Step 2
If you do not have an official place of work, check with your local compulsory health insurance fund which insurance company issues and replaces insurance policies.
Step 3
Contact the named company with your personal passport and expired policy. The passport must contain a registration mark. In some cases, you will also need a pension insurance certificate and work book. As a rule, it takes a few minutes to issue a new insurance policy with an insurance company.
Step 4
If you have official unemployment status and are registered with the employment service, contact your local Employment Center to replace your insurance policy. Employees of the employment service will check your documents and issue you a policy with a new validity period. In this case, the replacement of the policy takes place within a few days.
Step 5
For pupils and students, it is possible to extend the insurance policy both at the place of residence and at the place of study. To reissue the policy, contact the dean's office of the educational institution to the person responsible for working with insurance documents. Present a valid student ID and expired medical certificate. After completing the new document, get it in your hands.
Step 6
If at the time of the replacement of the insurance policy your social status has changed, that is, you have changed your place of work, place of residence or surname, be sure to indicate the new information in the application you provide to your employer, employment service or insurance company.
