Step 1
Item number 1. Clearly indicate the last name, first name, patronymic. For example: Ivanova Maria Ivanovna. If you changed the last name, first name or patronymic, be sure to indicate the old data, place and date of the change. For example: Petrova, changed in 1975 in Voronezh.

Item number 2. Enter the date, month and year of birth. For example: August 22, 1959.

Item number 3. Floor. For example: female.

Item No. 4. Indicate the place of birth, according to the data indicated in the civil passport. For example: Voronezh region, Zuevo.

Item number 5. Indicate the place of registration (zip code, city, street, house, building, apartment, telephone). For example: 123456, Voronezh, Voronezh region, st. Mirnaya, 12, building 9, apartment 6, phone 555-666-77.

Item number 6. Indicate citizenship. For example: Russia.

Item No. 7. Enter your passport details. For example: series 77 77 No. 987456, issued on August 15, 2001 by the Department of Internal Affairs of Moscow.

Item number 8. Indicate the purpose of obtaining a passport. For example: For temporary trips from the Russian Federation.
Step 9
Item No. 9. Indicate the reason for receipt (primary, instead of used, spoiled, lost). For example: instead of the used one.
Step 10
Pay special attention to items # 10 through # 13.
Since all documents and information specified by the applicant is undergoing strict verification, you do not need to try to deceive the competent state authorities, provide only reliable information, otherwise you will be denied the issuance of an RFP.

Item number 10. If you have ever been admitted to information that belongs to state secrets, be sure to indicate where, when and in what form it happened.

Access to state secrets is issued for a certain period, if at the time of registration of the passport it has not expired, its issuance may be refused.

Item number 11. Answer yes or no.
All applicants of draft age must provide a military ID or a certificate from the military registration and enlistment office in form-32.

Item No. 12. Indicate the presence or absence of a criminal record. If necessary, show a court order to drop charges against you.

Item No. 13. Answer yes or no.

Item No. 14. Indicate information about children under 14 years old in order to enter it in the APR.

Item number 15. Indicate where you worked, studied or served in the last 10 years.
Step 17
Item number 16. If you had a passport before, then indicate the series, number, date of issue and issuing organization. If you are getting a passport for the first time, then do not indicate anything.

Item number 17. Sign and date under the inscription: "I know that knowingly false information in the application may result in a refusal to issue a foreign passport."
