An application to the prosecutor's office is a written appeal from a citizen in which he informs about a committed or impending crime or a violation of his rights (the rights of other persons) both by state bodies and by any other persons (individuals or legal entities). The ability to correctly draw up a statement is very important, because this is the key to its early consideration and prevention of adverse consequences that have become possible as a result of violation of citizens' rights.

Step 1
Drawing up an application to the prosecutor's office must begin with filling in the so-called cap, which is located in the upper right corner of the sheet. It must indicate the name of the body or official (usually, when contacting the prosecutor's office, the application is written in the name of the prosecutor), to whom the appeal is made, his rank or rank, surname, name, patronymic of the applicant himself, address of residence (place of registration), contact telephone (if any).

In the center of the line, write the word "Statement". Then, in a free style, state the essence of your appeal (describe the situation that was the reason for writing the application). If possible, make up the text of the statement from small sentences, describe only the facts that matter. It is best to avoid ambiguous expressions and references to facts that you are not sure exist in your statement. If you have evidence to support your statement, please attach it to your application with a complete list at the end.
Step 3
After the main part is ready, state your request to take measures aimed at stopping the violation of your rights, restoring your rights, initiating a criminal case, etc. depending on the basis of the appeal.

Then, under the main text of the statement, be sure to indicate in a separate line that you have been warned of criminal liability for knowingly false denunciation under Art. 306 of the Criminal Code. Sign the completed application, indicate your surname and initials next to the signature, put the current date. Submit your application to the prosecutor's office. The fact of acceptance of the application is recorded by the issuance of a coupon, which indicates by whom and when it was accepted.
