Step 1

The pension certificate will allow you to receive free medical services. At your request, you can start saving money on the child's individual account to form his future pension.

To issue a pension certificate for children - apply for the form of the insured person's questionnaire to the branch of the Pension Fund of the Russian Federation in your city. If your child is attending a kindergarten or school, you can contact the accounting department of the institution to fill out the documents for obtaining this document.

Step 2

Fill out the ADV-1 form for a child. You can get clarifications on filling out the questionnaire at any branch of the Pension Fund of the Russian Federation. Fill in the data on the basis of an identity document (passport). Verify with your personal signature the correctness of the information specified in the questionnaire.

Step 3

Provide a package of the following documents to the territorial body of the PFR: a completed application form, a copy of the child's birth certificate, a copy of the passport of one of the parents (passport of a legal representative). If the documents and data provided by you are correct, you will be issued an insurance certificate within 10 days after the submission of the documents.
