Arrival sheet - a document confirming the fact that a person has arrived in another city. It also indicates basic information - where he came from and how far, and also indicates the place where he will live. That is why filling out the arrival form must be taken seriously.

The arrival sheet can be downloaded and printed from the Internet. Or you can take it from the local department of the FMS.

The information that is indicated on the arrival sheet must be accurate and clear. And most importantly - believable. The sheet contains a heading in which you need to indicate whose name you are filling out this sheet. As a rule, the address is indicated by the department of the FMS in the city or in the district of the city in which you arrived.

Next comes information about the person who arrived - name, surname, patronymic, date of birth, citizenship, place of birth. Next, you must indicate your gender and your registration address at the place of arrival.

After that there are terms where it is necessary to indicate the address at which you are registered as arrived. Next, it is prescribed on the basis of which document the identity of the arrived was established, i.e. passport data. The registration of the arrival sheet ends with the date and signature.
Step 5
On the reverse side, we indicate where the person came from, i.e. we write the address for registration. And then there is information, which is filled in by the specialists of the Federal Migration Service, that the information has been verified.
