Step 1

First, it should be noted that if the employee has not worked anywhere before, then you must issue a work book, since you are the first employer. Purchase work books from official distributors, only they register the series and numbers in the appropriate authorities.

Step 2

Fill out the work book with a ballpoint blue or purple pen. All entries must be legible and free of errors. Also note that entries are made only in the presence of the employee himself.

Step 3

First, you must indicate the last name, first name and patronymic of the employee according to the identity document. Remember, abbreviations are not allowed. Be extremely careful when writing data, if you make a mistake, you will have to write off the work book and start a new one.

Step 4

Enter your date of birth on the line below in the format dd.mm.yyyy. Then education, enter this information on the basis of a diploma of graduation or a certificate of secondary education. You do not need to indicate the institution itself, the name of the specialty in this line, just write "higher professional" or "secondary professional". If the employee has an incomplete education, then on the basis of a certificate from the institution or student ID, indicate “incomplete higher education”. On the next line, indicate the specialty of the employee, for example, engineer.

Step 5

Then put down the date of filling and give the work book to the employee to check all the data. After that, he must sign. Sign you below, then stamp the data with a blue round stamp in the place of "MP".

Step 6

If in the process of work an employee has changed his surname, then you must make changes on the basis of the relevant document, for example, a marriage certificate. To do this, cross out the old name with one straight line, and write a new one on top. Then, on the inside of the work book, enter the grounds (documents) for which the changes were made, write the position, surname and sign.

Step 7

In the event that all the sheets of the form are covered, supplement it with an insert. Be sure to indicate its series and number on the title page of the work book with the note "Issued insert".
