Birth statements of a child
To register the birth of a child, you need to fill out an application form No. 1 or No. 2 in the registry office. The first is filled in by married parents, and the second by a single mother. Form No. 2 is accompanied by an application form No. 3, on the basis of which you enter the name and patronymic of the child's father. This application is optional and must be completed by the mother of the child. There are other, much less commonly used forms of birth declarations, drawn up in relation to found children, children born outside medical institutions, etc.
The information required for the indication in the birth application is spelled out in family law. This is the personal data of the parents (last name, first name, patronymic, date and place of birth), their place of residence, citizenship, nationality (optional), details of their identity documents (passports), number, date of the marriage record, and also who made his body (for married parents). The parents also enter the surname, name and patronymic of the child, indicate the date of completion and put signatures. A medical certificate of the approved form is attached to the application.
Applications for registration, conclusion and dissolution of marriage
Persons wishing to conclude a marriage fill out an application according to form No. 7. In this form, in addition to other data common to many applications submitted to the registry office, the following data are indicated: age at the time of marriage, since the age of marriage is established in Russia; details of the record of the act of termination of the previous marriage in the event that it was registered; surnames assigned to spouses after marriage.
Divorce applications are submitted in Forms No. 8, No. 9 and No. 10. The first form is filled out by spouses who divorce by mutual consent; the second - by one of the spouses if the other is serving a sentence in places of deprivation of liberty for a term of more than three years; the third - by spouses on the basis of a court decision.
The legislation also approved the forms of applications for establishing paternity, adoption, change of name, death and other legally significant actions, including obtaining a repeated certificate, issuing archival certificates. They are filled out in the registry office, where you will be offered samples.
