If you need to obtain information from the act book or a duplicate of a previously issued certificate, you can apply to the registry office with a request. The simplest situation is when you yourself can appear at the registry office, which stores the act book with the information you need. In this case, it will be necessary to provide identity documents and the right to receive relevant information (for example, if a woman has lost her birth certificate, she will need to present, in addition to her passport, a marriage certificate, in which her maiden name appears).

If the registry office that performed the registration is geographically remote from the place where the applicant lives, then a written request is made indicating the closest registry office to the applicant and a request to send the necessary information and documents to the nearest registry office.

There is no established form of request to the registry office, although it is easy to find sample templates on the Internet by which you can make a request. After the nearest civil registry office receives the documents from the remote registry office, the applicant will be able to receive them with the provision of the above documents.

Step 5

In accordance with Article 333.26 of the Tax Code of the Russian Federation, a state fee of 100 rubles must be paid for the repeated issuance of a certificate of state registration of a civil status act, and 50 rubles for issuing certificates to citizens from the archives of the registry office.
