Based on the description of the current situation, make a calculation explaining the amount of the claim proposed for collection.
Step 3
In the summarizing part, state your request to the court to satisfy your request and list the requirements for the respondent, beginning the description of the appeal to the judge with the word "Please".
In conclusion, write the word "Application" and list all documents accompanying the application confirming the validity of your claims, as well as a receipt for payment of the state fee required for the application according to the law and a copy of the statement of claim for the defendant.
Put down the date of drawing up the application and sign, not forgetting to indicate the decryption of the signature.
