If an employee wishes to resign from the organization, he should write a statement asking him to dismiss him of his own free will. There is no unified form for this document, the application is written in any form, but it must contain the date from which the employee asks to fire him.

Instructions
Step 1
On an A4 sheet in the upper right corner, write the full name of the enterprise in accordance with the constituent documents or the last name, first name, patronymic of an individual in accordance with an identity document, if the organizational and legal form of the organization is an individual entrepreneur. Enter the position of the head of the company, his last name, initials in the dative case. Indicate your last name, first name, patronymic in accordance with the identity document, and the position you hold in accordance with the staffing table.
Step 2
Write the name of the document in the middle of the sheet with a small letter. In the content of the application, state your request to fire you of your own free will, indicate the date from which you should be fired. It must correspond to your last working day, when the corresponding entry is made in your work book, cash is issued against payment. But you are obliged to perform your job function in accordance with job responsibilities and timesheet. Write down the reason why you decided to quit voluntarily. The reason may be family circumstances, moving to another city, and the like.
Step 3
Put on the application a personal signature, surname, initials, as well as a date that corresponds to the actual date of writing the document.
Step 4
Your application is sent to the first person of the company for consideration. According to the law, the employer has no right to refuse to dismiss you, but can appoint a work-off. In the upper left corner, the director of the organization puts down a resolution in which he indicates the date of dismissal, puts a personal signature. The resolution looks something like this: “To dismiss from 22.11.2011. with working off for a period of two weeks. Ivanov. 08.11.2011 . Dismissal of an employee with working off is considered a dismissal by agreement of the parties in accordance with Article 78 of the Labor Code of the Russian Federation.
