The reporting form means a sheet of paper of a certain format, with the permanent information of the document indicated on it, as well as the space reserved for filling in the variable information. It is intended for further completion. The largest part of the company's documents is drawn up on the forms.
Instructions
Step 1
Indicate the document number. As a rule, a strict reporting form is called a receipt. Therefore, opposite this word, at the top of the document, put its serial number.
Step 2
Write the name of the customer (if it is a company) or his full name, if the customer is an individual entrepreneur or individual. For example, if you order a product from a company by filling out this form, then indicate in the "Customer" column the name of your company. Please fill in the address below for the location of the company. Please note that the order will be delivered to the address that you indicate in this column.
Step 3
Write the phone number by which the supplier company will be able to contact you to confirm the order or clarify some points.
Step 4
Fill the table. In the first column, list the names of the ordered goods or services. In the second column, indicate the number of conformity of each ordered product, according to the price list. In the third column, enter the units of measure for these orders. Next, indicate the quantity, that is, how many of each specific type of product you order. And in the last column, opposite each order name, write its cost.
Step 5
Print the total amount. To do this, calculate the cost of all ordered goods. Then write the resulting value in the very last line of the table.
Step 6
Fill in the company details. To do this, enter the information in the second small table, which is located on the right side of the form. In it, indicate the code according to the All-Russian Classifier of Management Documentation (OKUD), the planned date of the order and the service code. You will have to fill in the rest of the data in this table after receiving the goods (acceptance code and the actual date of order execution).
Step 7
Check all the specified data on the form to avoid mistakes. Then sign and indicate the amount of the advance payment (if you pay part of the order in advance). Next, write down the full remaining amount that you will need to pay after the delivery of the goods.
