Bonuses to employees for conscientious work are made by the employer (Article 191 of the Labor Code of the Russian Federation) on the basis of a proposal for incentives or the Regulations on bonuses in force at the enterprise. After consideration by the head of the submission, an order is prepared, for which the unified form No. T-11 for encouraging one employee and No. T-11a for two or more were developed and approved by the decree of the State Statistics Committee of Russia dated 05.01.2004.

Step 1

In the introductory part, in accordance with the accepted standards of document flow, write the initial details. This will be the full name of the organization. Next, fill in the fields indicating the number of the bonus order, the date and place of its execution. In the center write the name of the document "ORDER".

Under the title, briefly state the content of the order (on bonuses to the employee) and the grounds for its issuance.

Step 2

In the main part of the order, describe in full the grounds for the decision on the promotion. Further, after the word "I order", indicate the surname, name and patronymic of the awarded employee, his position and structural unit of the enterprise.

A separate item should determine the type of incentive (bonus, valuable gift, etc.) and the size (amount in words and numbers). There may also be an order for the accounting department regarding the transfer of the indicated amounts to the account of the wage fund or other source of funding.

In addition, at the end of the main part of the order, indicate the document that served as the basis for issuing this order (the idea of encouraging the employee from his immediate supervisor or the general Regulation on bonuses adopted in the organization).

Step 3

In the final part, leave a place for the personal signature of the manager, indicate his position and decipher the signature (surname and initials).

Below, place the place for the list of the employee, about whom the speech was in the order, after the wording "I have read the order." Here also indicate the position, name of the person to be awarded and the date of acquaintance.
