It is necessary
- - statement about the loss (theft) of the passport;
- - application for the issuance (replacement) of a passport in the form No. 1P;
- - 4 photographs of 35x45 mm;
- - original receipts for payment of a fine and state duty;
- - an extract from the house register confirming your registration.
Instructions
Step 1
Contact the department of internal affairs at the place of registration or at the place of theft with a statement about the loss or theft of your passport. The police department has its own file with citizens' applications for the issuance of passports. If the necessary information is not in the card index, you will need to provide any of your documents - a birth certificate, driver's license, work book, etc. After consideration of the application, you will be given a coupon notification of registration of a message about the incident.
Step 2
Contact the Federal Migration Service (passport office) at your place of residence with a police certificate. Write a statement addressed to the head of the passport office indicating the circumstances of the loss of your passport. For the loss of your passport, you can be brought to administrative responsibility and a fine.
Step 3
Pay the fine and state duty at the Sberbank branch according to the receipts issued at the passport office. Take four color or black-and-white photographs in the fixed size 35x45 mm. Issue in the appropriate organization an extract from the house register confirming your registration.
Step 4
Go back to the passport office with all certificates, receipts and photographs. Fill out the application form for the issuance (replacement) of a passport in the form No. 1P. Go to the OVD unit on the appointed day for a new passport. The term for consideration of an application for the issuance of a document is from ten days to two months from the date of its submission.
